The Census History Form is 8 ½ x 14 inches in size and folds to 8 ½ x 11 inches. It is used for recording up to ten census years of basic data for one family on a single page. Data space is for the year of the census, place of the census, age and birthplace of the person on the census. The form has spaces for father, mother, and up to ten children.
